This simple Feedback module allows you to collect general information from your users.  You have the ability to create categories and subjects as well.  If you would like feedback displayed on your site, you can allow all or moderate certain submissions.   You may require the Name field.  In addition, CAPTCHA is an option to help reduce the number of automated submissions.
